About this service

Our MHT and MHT+ teams provide community-based support and treatment for adults of all ages experiencing mental health challenges. These teams bring together the expertise of the former Community Mental Health Teams (CMHT) and Community Mental Health Services for Older People (CMHSOP) to make sure that care is tailored to individual needs, regardless of age

Who we support

We offer assessment, treatment, and ongoing support for adults facing moderate to severe mental health difficulties. This includes:

  • people aged 18+ with complex mental health needs requiring specialist intervention

  • older adults with mental health conditions complicated by age-related needs

  • individuals of any age with dementia-related needs

  • support and advice for carers, professionals and care homes

What to expect

After an initial assessment, we work with you to develop a personalised care plan. This may include:

  • further appointments with our specialist team

  • group sessions and therapy options

  • neuropsychological assessments

  • support for carers and loved ones
