The Intensive Home Treatment Team for Older People (IHTT) assesses and cares for older people with more acute and complex problems related to their mental health or dementia.
It provides intense support to help avoid hospital admission if that is what the person prefers. This means the team will normally offer support where a person lives. The team helps to support people when they are discharged from hospital and also helps to prevent the breakdown of a caring situation at home or other place of residence.
How to refer - For external referrers, such as from GPs, adult social care, Leeds Community Healthcare, Leeds Teaching Hospitals etc., please access the IHTT via our Single Point of Access. Please ensure that all urgent referrals are called through to the team on Freephone 0800 183 1485.
Internal LYPFT referrals from CMHTs, Inpatients, Care Homes Team and Liaison Psychiatry should be discussed with the shift coordinator (where possible) and then referred on electronically to the IHTT duty desk.
