Our services provide physical, psychological and/or spiritual support, and are free of charge. We’re available day and night to ensure that you have the right care, at the right time.
When someone is ill, it’s not just the patient who is affected. Life can be turned upside down for the family and for friends and carers. That’s why we’re here for you, for as long as you need.
Our 24hr advice line on 0345 0130 555 is available for patients, their families/carers and healthcare professionals who need immediate medical advice on someone’s care. It is run by experienced nurses who have round-the-clock access to specialist medical advice from our own doctors.
To make a referral or to find out more information about our services, please ring the Clinical Coordination Centre (Mon-Fri 8-6pm | Sat-Sun 9-5).
