About this service
The intermediate mental health & physical care team is a joint mental health and physical health service which aims to bring a complete package of healthcare treatment under one team.
The team is a specialist service for older people who live in Westminster. The service provides intensive targeted treatment to help patients recover at home rather than be admitted to hospital. In addition they also work with hospitals and mainstream services to support patient when they return home.
How to make a referral:
We receive referrals from professionals involved in your care, for example your GP.
