Referral to Bury Home Treatment is an alternative to hospital admission. We provide monitoring, treatment and support to you in your own home.
This includes monitoring the effects of your medication, a risk assessment and a mental state examination on each visit.
If your level of risk escalates, in any of the four domains, or mental state deteriorates to the extent that an admission is required - this can be facilitated by the team. We will implement a supported discharge wherever possible, to reduce the length of your stay.
Mental health professionals can refer to us after completing a mental health needs and risk assessment.
Referral should be made to the gatekeeper.
