We provide intensive support in the community, for people aged between 16 and 65 years, who are experiencing an acute mental health crisis as an alternative to admission. The team will also provide intensive support to service users who are leaving a hospital bed as part of an individual’s discharge pathway.
Our treatment includes brief psychosocial interventions, risk management and medication monitoring.
Mental health professionals can refer to the team following completion of a mental health needs and risk assessment.
