About this service

The Trust's Mental Health Home Treatment Teams (MHHTTs) provide an alternative to inpatient care by offering short-term intensive community support. They are committed to working with service users and carers to find solutions and prevent relapses and all individuals are treated with respect, dignity and honesty. The aim of the service is to assertively engage with service users in mental health crisis while minimising the degree of disruption to their lives and offering clear information to promote service user choice. The teams consist of medical staff, nurses, social workers, occupational therapists, pharmacists and support workers. All service users under MHHTT will be allocated a Named Worker who will oversee their care whilst under the team. Service users will be offered a comprehensive assessment of their needs with individualised care plans being developed with them in order to address these needs. MHHTT will engage with other services involved in a service users care (for example CMHT) in order to develop joint care plans.
