About this service

The Crisis Resolution and Home Treatment Team provides a rapid assessment of people who are experiencing a mental health crisis of such severity that without the intervention of the team, a hospital admission would be necessary. Adults from the age of 18 who without this service would require hospital admission.

Referral (how to access)

We accept all mental health diagnoses – *please see exclusion criteria.

Referrals:

  • Focus line

  • GP

  • healthcare professional

  • LAMP

  • Network for change

Referrers contact 0300 300 1010 (open 24 hours) and press option for referral, they will be put through to a call handler who will take details and pass to a triage nurse. A telephone triage (assessment) should take place within 2 hr of receipt of referral, this will vary dependent on time of day and volume of referrals, if a face to face assessment is the outcome of the triage assessment then this will take place within 24 hours

*Criteria exclusion: We do not accept referrals where the primary diagnosis is: drug and alcohol abuse dementia.
