About this service

The services provides assessment and short term treatment in the community for people who might otherwise need admission to an acute psychiatric unit. It offers support to those who have been in hospital and are ready for early discharge. Assessment will normally be in the individual's home or at an NHS unit. While the service user is receiving support from the team, they and their carers can contact them 24 hours a day, seven days week. The team will provide intensive support and care during a crisis, according to an agreed plan of care. The care plan will include the location where the team will meet the individual, how often the visits will happen, interventions needed, contact details, medication/treatment information, and the needs of families/carers. A 24-hour / seven-day-a-week community based team providing assessment and home treatment as appropriate for people experiencing a mental health crisis. Referrals to the team are made by other mental health services including the First Response Service.
