The service provides assessment and short-term intensive treatment (up to 6 weeks) for older people experiencing a major mental health concern that may otherwise require a period of hospital admission.
The team comprises a consultant psychiatrist, advanced nurse practitioner, community psychiatric nurses, and mental health support workers.
We also provide a specialised short-term care pathway (up to 2 weeks) called dementia intensive support. This offers a holistic care approach to people experiencing severe behaviours and psychological symptoms of dementia.
We promote independence and recovery with older people and their carers to an agreed care plan which aims to offer a high level of support at home, or in a care establishment. For people who have been in hospital we can help facilitate an earlier discharge by offering a short period of supported discharge at home.
For people who require continuing or longer-term support we can refer to other pathway teams within older peoples mental health esrvices. This ensures patients receive long-term treatment and therapy as appropriate.
Open 8am - 8pm every day, including weekends and Bank Holidays.
