About this service

The Central Norfolk CRHT is a team of experienced mental health staff.

The service provides assessment and short-term, intensive community support.

Our CRHT team aims to work with people to prevent hospital admission. They also provide intensive support to hospital patients to aid discharges from hospital.

Our service acts as a gatekeeper to other mental health services. This includes acute inpatient care. Our team tries to prevent hospital admission by exploring all potential alternatives.

If needed, the Central Norfolk CRHT also refers people to other services. Usually for longer term treatment.

People who referred to this service see a mental health professional. Either at our base at Hellesdon Hospital, in their own home or a suitable community setting.

Our team carries out an assessment with the person and their carer or supporter and agree a plan. This may involve ongoing support from the CRHT for a short period of time. Or it could involve a recommendation to the referrer for a plan of care.  Signposting to a more suitable service is also an option or, in some cases, admission to hospital.

Our Central Norfolk CRHT can only work with people who consent to our service.

This service is provided by the NHS.

Referral (how to access)

Referrals accepted from:

  • Secondary NHS mental health services

  • Specialist mental health services

  • GPs and Primary Care Practitioners

  • Other health services

  • Emergency services – including police and the ambulance service

  • Agencies other than health services which support people with mental health problems

  • Those known to the CRHT, including their families or supporters

  • Mental Health Liaison and Diversion services (either via court or Police Investigation Centres)

  • Inpatient acute admission wards

  • Emergency duty teams (Social Services)

  • Mental Health Liaison via hospital Accident and Emergency

  • Service users known to secondary mental health teams. This is out of usual working hours, as an alternative to hospital admission.
