The Home Treatment Teams work closely with the community mental health teams to enhance the care provided in the community for unwell adults and older people.
The service enables people to either avoid hospital admission or allow them early discharge from hospital. Home Treatment helps people to remain at home whilst they recover.
This service is by referral from secondary services only and operates between 9am-10pm every day.
