About this service
Brent Community Learning Disabilities Team is made up of health and social care professionals from Brent Council and the NHS.
It provides specialist health support to enable adults with learning disabilities aged 18+ to be healthy, independent and valued members of their community. The team includes nurses, occupation therapists, speech and language therapists, social workers, psychologist and psychiatrists, who undertake a number of health and social care assessments and are committed to supporting carers of residents with Learning Disabilities.Carers may self-refer, or request referral from their GP, Adult Social Care and Children Services.
