About this service

The Bromley West Locality Mental Health Service delivers evidence-based interventions that help service users recover from mental health conditions. We aim to: - Help service users move along and across treatment pathways whilst maintaining the best quality of care - Improve service users’ capacity to care for themselves - Work closely with primary care, the voluntary sector and other partner organisations - Reduce the need for inpatient care and maximise the use of community-based services This service combines mental health teams from across Oxleas to provide a local care service. These teams include: Bromley Mental Health Hub (BMHH) - BMHH is the single point of contact for all mental health referrals and a direct link between primary and secondary care services. It focuses on screening, tele-triage, providing support to GPs, and directing service users to the most appropriate care pathway. As part of the Community Transformation Plan, This team is integrated with BLG Mind. The Anxiety, Depression, Affective, Personality Disorders & Trauma (ADAPT) Team - The ADAPT Pathway provides focused, therapeutic interventions to adults who require care and treatment for anxiety, depression, affective disorders, personality disorders and trauma. The Intensive Case Management for Psychosis (ICMP) Team - ICMP provides care and treatment for service users with a diagnosis of schizophrenia or bipolar disorder. To access this service, you must be: - Aged between 18 and 65 - Registered with a Bromley GP We do not accept referrals for patients who: - Are eligible for Adult Learning Disability Services - Have eating disorders that require tertiary intervention All referrals should be sent to the Bromley Mental Health Hub (BMHH) email provided in the contact section. Referrals are screened within 24 hours and identified as either Urgent, Priority or Routine. Urgent referrals are contacted for triage within 24 hours. Priority referrals are offered a triage within 7 days. Routine referrals are contacted within 14 days. The service is not available on bank holidays. There are no out-of-hours arrangements in place for CMHT. However, patients needing urgent support can access this through the crisis care pathway. Each team has clear contact details that are available on the Oxleas website. Patients are given contact details through the care planning process.
